Your first eye visit:

We would like to make your first visit to Dr. Burger, a smooth and pleasant experience.

Please bring the following:

  1. Copy of your ID or driver’s license card
  2. Copy of your Medical aid card
  3. Your referral letter – only if you have received one.
  4. Your pertinent, previous medical records – write this on a piece of paper.
  5. List of medications and dosages – write this on a piece of paper and/or bring your medication along, especially your eye drops.
  6. Arrange a driver or public transport – Your eyes may need to be dilated which could impair your driving ability for 2 – 6 hours. A pair of dark glasses may also help you here.

    Also be prepared to remove your contacts or glasses during the exam. Tests that use dye (fluorescein) to temporarily color your eyes may permanently discolor your contact lenses.
  7. There are 2 forms to complete:
    • Patient Registration Form and
    • Medical History Questionnaire
  8. You can complete these forms prior to your visit by downloading them and printing them in the PDF format below.

    Alternatively you may choose to complete these forms at the start of your visit. Please arrive 20 minutes early to give sufficient time to allow for accurate and sufficient data capture.

    Please allow two hours for your initial appointment as Dr. Burger has the ability to conduct many tests onsite. This will help him to determine the health of your eyes quickly.

    Please also note that an eye emergency may walk through the door at any given time, which could delay you appointment.

    Dr. Burger is well aware of the fact that time is important to you and he always aims to keep to the elective eye appointment times.
